samlet vurdering prioriteres patienterne.<\/li>\n<li>Der laves symptomregistrering og behovsvurdering.<\/li>\n<li>HospiceS\u00f8nderjylland melder umiddelbart tilbage til henvisende l\u00e6ge og patienten, hvorvidt patienten er visiteret til ophold p\u00e5 HospiceS\u00f8nderjylland eller ej.<\/li>\n<li>Ved ledig stue eller &#8220;forventet snarlig ledig&#8221; stue tages kontakt til patienten. Det er patientens nuv\u00e6rende situation, der er afg\u00f8rende for indl\u00e6ggelse.<\/li>\n<li>Der indhentes eventuelt nye, relevante oplysninger pr. telefon.<\/li>\n<li>Indl\u00e6ggelsesforl\u00f8bet p\u00e5 HospiceS\u00f8nderjylland vurderes j\u00e6vnligt.<\/li>\n<\/ul>\n<p>Henviste patienter prioriteres dagligt ud fra en samlet behovsvurdering.<\/p>\n<h3>Ved eventuelt udskrivelse<\/h3>\n<ul>\n<li>Udskrivelse foretages i samarbejde med hjemmeplejen, egen l\u00e6ge og evt. palliativt team ofte ved f\u00e6llesm\u00f8der med patient og familie.<\/li>\n<li>Egen l\u00e6ge modtager epikrise fra hospicel\u00e6ge.<\/li>\n<\/ul>\n<h3>Ved d\u00f8dsfald<\/h3>\n<ul>\n<li>Egen l\u00e6ge orienteres skriftligt af hospicel\u00e6ge.<\/li>\n<li>Kommunens visitationsafdeling\/hjemmeplejen orienteres skriftlig af HospiceS\u00f8nderjylland.<\/li>\n<\/ul>\n<p>Vi ser frem til et godt samarbejde, og du er meget
